The Importance of Proper Diagnoses: Righting a Surgical Wrong

Two years ago, Mr. J noticed pain in his right groin area. He went to different clinics and hospitals for consultation – he was seen by an orthopedic surgeon, urologist, general surgeon, gastroenterologist and a physical therapist. His symptoms, however, did not improve.

After losing faith in the medical system in Shanghai, he decided to go back to the US to seek help. Doctors in his hometown diagnosed him with an umbilical hernia and performed umbilical hernia repair surgery. After the surgery, and despite using powerful pain medication, he was still in pain. 

Now there was abdominal pain in addition to the original groin pain. A simple sneeze would cause him to whimper in pain for the entire day. To reduce the pain, he was forced to walk like an old man with his back hunched. And the abdominal pain woke him up at least three times per night, leaving him feeling miserable every morning.

After hearing his friend had had hernia surgery at Shanghai Yosemite Hospital (JingAn) and was able to recover fully in just a few weeks, Mr. J decided to come in for a consultation. The team of doctors paid close attention to his medical history and reviewed his previous medical documents closely. They identified a possible cause to his ailments and tried conservative treatment. His groin pain improved greatly, but the abdominal pain persisted. 

During the initial consultation, the team of doctors deduced that his abdominal pain was related to the surgery he received in the US. More than 15 tacks had been used in the hernia repair. The location of the abdominal pain showed a high correlation to where the tacks were. So the team of doctors proposed surgery to remove the tacks that were causing the nuisance.

Placing tacks is easy, but removing them is hard. Right before the surgery, an X-ray was performed to locate the tacks that were causing the pain – the team of doctors marked where they were on Mr. J’s abdomen. 

During the minimally invasive laparoscopic surgery, intestinal adhesion was observed and fixed immediately. After the abdominal wall came into plain sight, the hernia patch was carefully examined. The tacks, which had burrowed into the abdominal wall, were barely visible. 

While the surgeon was using very sophisticated tools, there aren’t any tools designed to remove tacks. The surgeons had to combine their skill and finesse with different instruments to slowly unscrew the tacks from Mr. J’s abdomen. After operating for three hours, the surgeons had successfully removed the adhesion, as well as removing seven troublesome tacks.

The next day, Mr. J recovered well and was able to walk with his back straight for the first time in two years. He couldn't stop smiling from joy as he enjoyed the sunshine during an outdoor stroll. Upon satisfaction of the doctors, he was discharged from the hospital that afternoon.

Intestinal adhesion is a common complication after abdominal surgery. It can manifest itself as abdominal pain, bloating, nausea and vomiting. In some cases, it can lead to intestinal obstruction. Intestinal adhesions can initially be managed conservatively. Once severity has increased, surgical intervention is usually required.
